"February 21, 2012 is the deadline to register to vote in the Primary Election. Chicago voters may register for the first time - or update their records if they have moved or changed their names since they registered:
- by submitting this mail-in form. Use this form to: register for the first time; update your address; or update your name.
- in person at 69 W. Washington St., Sixth Floor.
- at any Secretary of State's office where driver's licenses and state IDs are issued.
- through an active deputy registrar affiliated with a local organization, such as a political party, ward organization or other state-certified body."
